VACCINE ROLL OUT INFORMATION

Once you sign up at this link, you may be contacted to schedule an appointment. Signing up on this website does not guarantee a vaccine will be available; you are placed on a standby waiting list. You must wait to be contacted to schedule an appointment. If you previously signed up through this link, you do not have to sign up again.

COUNTY PARATRANSIT will provide free transportation to all seniors and disabled constituents who need a ride to their vaccination appointment.

  • All reservations must be made at least 24 hours in advance, in order to allow time for scheduling
  • To make a reservation, residents can call 914-995-7272.

Vaccine doses continue to be  limited and unfortunately, we are not able to accommodate everyone who would  like to schedule an appointment.  We encourage eligible individuals to continue visiting the NYS Department of Health website, and Northwell’s Vaccination Hub online, for  available appointments at other sites throughout the state.
      
We are hopeful that we will  receive additional doses for community vaccination and will continue to keep  you informed if and when we are able to open additional vaccination dates.     

Vaccine Rollout Eligibility and Phases:

--Vaccine Form:
Before receiving vaccination, eligible New Yorker must complete the New York State COVID-19 Vaccine Form; more information is available at 
https://covid19vaccine.health.ny.gov/ 
and then click on CHECK ELIGIBILITY.

Step 1: Determine eligibility and schedule an appointment. First, you should complete Am I Eligible? App to check their eligibility and find vaccination locations near you where you can schedule an appointment, or call the New York State COVID-19 Vaccination Hotline. The COVID-19 Vaccination Hotline is open 7AM - 10PM, 7 days a week, for scheduling vaccination appointments for eligible New Yorkers: 1-833-NYS-4-VAX (1-833-697-4829).Note that due to limited supply from the federal government, it may be 12 or more weeks before an appointment is available to you.  

Step 2: Complete the Vaccine Form. Once and only once you have a confirmed appointment, you must complete the New York State COVID-19 Vaccine Form. This form can be completed online and you will receive a submission ID, or can be completed at your vaccination site. (We encourage New Yorkers to complete the form ahead of time.)

Step 3: Bring proof of eligibility to your appointment. When you go to your vaccination site for your appointment, you must bring proof of eligibility. This may include an employee ID card, a letter from an employer or affiliated organization, or a pay stub, depending on the specific priority status. If you are eligible due to age, you should bring a form of ID (like a Driver's License or passport) that includes your date of birth.

COVID-19 TESTING FACILITY:

There is a testing facility located at 265 Route 202 (the Old Chase Bank).
